SSRIs are the most important component in the treatment of depression as well as other mental health issues. All SSRIs do not have the same qualities, and when reviewing a patient's case, be sure to be aware of specific factors that could put patients at risk of problems resulting caused by particular SSRIs. Here are some reasons why you should avoid SSRIs.
QTC Prolongation
If you suspect that your person is at risk of QT prolongation or other medications that can cause QT prolongation issues, it is recommended to be cautious about using the drug citalopram. It's among the most dangerous medications, particularly when we increase doses.
Anticholinergic Burden
Paroxetine is not often utilized in patients with geriatric conditions, and the reason why this is the case is because it is more anticholinergic than the other SSRIs within the class. Beware of this medication in older patients as well as those who are taking other anticholinergic medicines.
Diarrhea
All SSRIs have the potential to cause GI adverse reactions; however, sertraline is more likely to cause adverse effects than others in the case of diarrhea. For patients suffering from gastrointestinal issues that can cause diarrhea, it's advised to avoid sertraline. It is sometimes referred to "Squirtraline" due to its tendency to cause loose stool. IBS, Crohn's as well as Ulcerative Colitis are common GI problems that can trigger diarrhea-related symptoms. It may be beneficial to look into a different SSRI rather than sertraline.
Many drug Interactions
Fluvoxamine is a well-known drug for its drug interactions, and that is the main reason you won't find the drug used so often for patients with major depression. It is crucial to stay away from the use of this medication. This is particularly true for polypharmacy patients.
CYP2D6 Interactions
Fluoxetine and paroxetine are acknowledged to have an impact on the CYP2D6 enzyme. A major and serious interaction between these drugs is tamoxifen. The addition of paroxetine or fluoxetine to the regimen of a patient who is taking tamoxifen could decrease the efficacy of the tamoxifen.
Insomnia
Fluoxetine (and possibly sertraline) are more stimulating. This can be beneficial in those who feel numb and rest all throughout the day, especially when they experience depression-related symptoms. For those who have problems sleeping, these medications could make it more difficult to sleep. If you observe someone who is using a sleeper drug and plans to begin with an SSRI, it is best to stay clear of these two activators.
